A berzerker using a hammer in their off-hand slot.
1. Berzerkers are supposed to only be able to use left-hand usable axes in their off-hand slot.
2. It may be only this one item, it may be more. I'm not a zerker so I don't know.

Make all items except left hand usable axes, not able to be used in Berserker's offhand slot.
The guy in this picture says it's a DF sapphire hammer. Not sure if that's true or not, who knows but maybe check that one first.
If it is actually that hammer then look at Superior Accursed Avernal Hammer from DF merchants.
