Forlorn chain drops from Mithra was reported in issue #1385 to drop with various qualitys. Team adjusted the quality to 89% which is incorrect for the patch of 1.65.
1. Kill mobs in Tomb of Mithra until a Forlorn item drops
2. Look at quality
Forlorn chain should be adjusted to 80% quality which was the quality the items had from start and beyond 1.65 and 2003.
